2. Registration of Births outside Sri Lanka

2.1.Application for registration of a birth outside Sri Lanka under the Ceylon Citizenship Act  NO 18 of 1948.

2.2.Particulars  for Registration of a Birth -Form B  144 (if the Application for registration of birth is made within three months of the child's birth) .

2.3.Declaration of Birth After Three Months (Section 24) – Form B 6 ( if the Application for registration of birth is made after three months of the child's birth).

2.4.Declaration that the applicant (father/mother) has not acquired citizenship of a country other than    Sri Lanka at the time of child's birth.

3.1. Notice of Marriage (Form B - 100).

4. Registration of Deaths

4.1.Declartion of Death  under Section 30 Registration  B 11 (if the Application for registration of death  is made within three months of the death) .

4.2.  Declaration of particulars relating to the death for purposes of late registrations (Section 36) – Form - Registration B 15 (if the Application for registration of death  is made after  three months of the death ).
