Under Section 7 of the Consular Functions Act, the Embassy exercises the power to register a birth of a child born to Sri Lankan parents, in Italy, Greece, Cyprus, Albania and Malta.
The application for the registration of birth must be made within three months of the birth.
The application form for the registration of birth could be obtained over the counter at the Consular Division of the Embassy or by post, by sending a self-addressed stamped envelope with a request. The applications can also be downloaded.
When a child is born abroad to a citizen of Sri Lanka, there is an additional requirement under the Citizenship Act that such birth should also be registered within one year at the office of the diplomatic representation of Sri Lanka in the country of birth or at the Citizenship Division of the Ministry of Defence in Colombo . Therefore, when an application is made for the registration of a birth under the Births and Deaths Registration Ordinance, an application for the registration of the child under the Citizenship Act should also be made.
When an application for registration of a birth is made one year after the birth of the child, such applications are forwarded by this Embassy to the Registrar General's Department, Central Record Room Maligawatta, Colombo 10, Sri Lanka, Tel : 00-94- 11 23 29 773,Fax : 00-94 -11 24 33 075 for an approval. In such cases the birth certificate is issued only after approval is received from the Registrar General in Sri Lanka.
If the Birth certificates are required in English, it is advised to fill the applications in English.

Duly filled Particulars for Registration of a Birth -Form B 4 (if application is submitted within 3 months of the child's birth) .
Duly filled Declaration of Birth after Three Months (Section 24 -B6) (if application is submitted after 3 months of the child's birth) .
Original Birth Certificate of the child in which the names of both parents appear (Birth Certificate has to be issued by the competent authority in the country where the child was born )
Declaration that the applicant (father/mother) has not acquired citizenship of a country other than Sri Lanka at the time of child's birth. Click here to download application.

Permit of Stay (SOGGIORNO) Parents, if available
Original Birth certificates of the parents.
Original Marriage certificate of the parents. (If the marriage has been registered outside Sri Lanka it should be countersigned by the relevant diplomatic representative of the country concerned)
Citizenship Certificate of parents if they were born outside Sri Lanka
If the child's birth was not registered within one year of the birth, a letter signed by both parents, giving reasons as to why it was not registered within the first year of birth
An affidavit from the father relating to paternity of the child if parents are not legally married at time of child's birth.

Upon Registration of the Birth, the first copy of the Birth Certificate would be delivered to the applicant over the counter and the second copy will be sent to the Registrar General's Department, Colombo, for record purposes.
The Embassy does not issue additional copies of Birth Certificates. Applicants who wish to apply for additional copies should request them from the Registrar General's Department in Colombo, Sri Lanka. The Embassy issues only a certified copy of the birth certificates after charging relevant fee.
Obtaining a fresh copy of birth, marriage or death registration registered at the Embassy of Sri Lanka in Rome.
1. Duly completed application form (see below)
2. Passport of applicant, supported by marriage, birth certificate to establish direct connection to the those listed on the Certificate. ie: if a fresh copy of a child's birth certificate is being requested, the parents certificate of marriage along with the Passports.
