Application forms can be obtained from the Consular Division of this Embassy by sending a self addressed, stamped envelope (9' x 6') with a request letter. The applications can also be downloaded from this website.

A Sri Lankan Passport initially issued for a period of five (05) years may be extended for a further five (05) years. A Sri Lankan Passport initially issued for a period of ten (10) years, cannot be extended further.

Applications for new passports can be made in advance of the expiry of the current passport.

The Sri Lankan religious dignitaries who are living in Italy representing the Buddhist, Hindu, Islamic and Christian faiths could certify the new passport applications along with their seals.

The applicants who lost /damaged the passports are required to call over the Consular Division of this Embassy personally and face an interview to handover the applications.

Applicants who are living in Greece and Cyprus must personally hand over their applications along with the supporting documents to the relevant Sri Lanka Honorary Consulates located in those countries.
