Required documents: All documents should be submitted in original with two photocopies.

b . 4 colour photographs [3.5 x 4.5 cm] taken within 3 months (Direct Face)

c. Current Passport  and photo copy of data page and alteration pages

d. Original Birth Certificate (Translations are not considered as originals)

e. Sri Lankan National Identity Card

f. For female applicants, in case of the change of the maiden name after marriage, original Marriage Certificate. If the marriage has been registered outside Sri Lanka, such Marriage Certificate should be countersigned by the diplomatic representation of the country where the marriage was registered.

g. If the applicant is a Sri Lankan citizen by registration, original Citizenship Certificate/Dual Citizenship Certificate.

h. If there is a change in the profession or new profession to be included in the passport, original professional certificate which was certified by the Consular Division of the Ministry of External Affairs in Colombo.

i. Relevant fee in Euro
